Prostate Cancer Center (Men’s Health)

Have you only been told about two options: an operation and/or radiation therapy? That is not the full picture. There are many treatment options for prostate cancer – and new ones are being developed in an ever shorter interval.

Our concept

“Why remove the whole prostate when the cancer is just in one area?”
We at the Prostate Center will use novel techniques for diagnostic and treatment planning, which are based on new scientific publications or from up-to-date discoveries of leading American clinics (i.e. John Hopkins University Hospital, Texas Medical Center, Mayo Clinic). We at the Prostate Center offer novel diagnostic and therapy modalities, as soon as they have general approval (FDA, CE). Among them is the treatment of prostate cancer with the innovative treatment of Irreversible Electroporation (IRE, NanoKnife) which is a gentle yet effective replacement of surgery.

Diagnosis

Our up do date diagnosis starts with the new Prostate MRI: Non-invasive, painless and precise.

When multimodal and three-dimensional MRI images are used, cancer loci in the prostate can be identified and localized with pinpoint accuracy and a detection sensitivity of at least 85% were no need for biopsy.

A prostate MRI also shows whether the carcinoma is only present in the prostate or has infiltrated nearby tissue (local staging). Ultrasound, computed tomography (CAT scans) or PET/CTs are not able to do this.

In most cases no biopsy is needed with this high sensitivity 85% MRI

MRI-guided 3-d biopsy: Painless, accurate, no risk of infection

If a biopsy is necessary, we use a short-acting anaesthesia and sterile operating conditions at the Prostate Center, which eliminates pain and the risk of infection. To ensure that we have found all of the carcinomas, we use ultrasound to determine the location of all places on the MRI images that seem suspicious (MRI and ultrasound images are shown simultaniously).

3-d mapping biopsy and histomechanical modeling

In some cases, more small samples are systematically taken over the rest of the prostate at 5-mm intervals. Every sample is localized three-dimensionally so that they can all be joined together after the pathological analysis to a 3-d model of the prostate using histomechanical modeling. This model shows the exact distribution of the cancer foci in the prostate and the Gleason score of each. If necessary, the model is overlaid with ultrasound and/or MRI images and serves as the basis for planning the focal, prostate-preserving therapy. Histomechanical modeling is a combination of two procedures, each with >85% sensitivity, a quantum leap in precision compared to the usual procedure.

MRIs and 3-d biopsies work hand in hand to produce a complete histological map of the entire prostate

Elevated PSA Level

Certainty with modern diagnostics – even without invasive procedures

A person's PSA level is at best an alarm signal for cancer, but not confirmation of a diagnosis. Less often, a carcinoma is responsible for the elevated PSA level. And the PSA test is not without its problems: Too often, it raises an alarm that leads to an unnecessary treatment such as a biopsy, and biopsies themselves are not very reliable

With high quality Magnetic Resonance Imaging (MRI) and the newest biochemical and imaging procedures, we can help you to be sure – in many cases without a biopsy.

Good reasons for diagnosis at the Prostate Center

  1. Reliable cancer detection: Rectal punch biopsies only detect 30-35% of all carcinomas, but we have detection rates of 85-100%.
  2. Reliable assessment of the danger of the carcinoma: the Gleason score can only be determined correctly with a 3-d biopsy.
  3. Avoiding unnecessary biopsies: If a prostate MRI is negative, it is often possible to forgo a biopsy.
  4. Avoiding prostate infections: We work under sterile operating room conditions to eliminate pain and the risk of infection.
  5. Reliable basis for the right therapy: Our diagnostic method finds and characterizes all cancer foci in the prostate, thus providing a reliable basis for choosing the best type of therapy, from "active surveillance" through focal therapy to operation/radiation.

You no longer have to leave decisions that influence your life to chance. Make sure you have the right basis for the right decision!

Treatment

NanoKnife® Therapy

Treating prostate cancer – no pain, impotence or incontinence; a one-time, one-day (24 hours) treatment, no rehab or aftercare necessary, no scars or radiation damage, no hindrance to follow-up therapy. Until just recently, this was unthinkable. Now, it's reality.

In the last three years, we at the Prostate Center have become the world's leaders in developing the irreversible electroporation (IRE, NanoKnife®) method until it is ready for use to treat prostate cancer. More than 300 successfully treated patients can confirm this.

Unlike all other treatment procedures, NanoKnife® only destroys cancer cells; vital tissue structures are not affected. This makes NanoKnife (IRE) the first tissue-selective form of therapy and the beginning of a new generation of therapy.

Current treatment methods lead to impotence (loss of erection ability) and incontinence (loss of bladder control) in the majority of men. These side effects can usually be avoided by using NanoKnife® treatment.

Other treatments at our center:
  1. Immunotherapies
  2. HIFU
  3. Laser thermotherapy
  4. Cryotherapy
  5. Photodynamic therapy (PDT)
  6. Hormone therapy
